configure line with qmailadmin:
--enable-spam-command='|preline /usr/bin/spamc -e /usr/bin/maildrop
If someone is using the setup also.

How big is the attachment? If the mail is over 250k it may be bypassing
SA entirely. What do the headers say? Is there any SA markup?
