I happen to have the same problem as Eric where the self test fails on a brand new machine (SA, perl modules fresh installs and not upgraded from anything). As it happens I do use sendmail and will have to add the sendmail.cf change you mentioned, but is this actually needed for the self test to work? Unless the self test is broken, it would seem like something else is also going on.
