I suppose tricky plurals are handled well with https://solr.apache.org/guide/solr/latest/indexing-guide/filters.html#hunspell-stem-filter Medical terminology it's the whole industry. Perhaps you were lucky to get something from https://www.nlm.nih.gov/research/umls/licensedcontent/umlsknowledgesources.html
