When you start fresh with a new system/component like a hard disk, it is proper
to firstly format it *entirely* - right at the very beginning from live-cd
(forget about your previous installation), with live-cd tools (cfdisk, gparted,
etc).
You may discover some facts about the disk, partition table(s), disk's health,
effective size (after formatting) vice nominal size, any bad sectors present
(original, by manufacturer) and their possible remapping to other/healthy
sectors, etc.
After formatting, reboot (due to MBR setup), and view the hard disk again as
posted before (dmesg, cfdisk, fdisk -l), also from live-cd. You may post
the output here ('fdisk -l' in particular).
If all looks good (see above) you can perform your installation once again with
custom layout.
