Not sure if this is relevant to your case, but It might be.
Does your KVM have a EDID emulator?
EDID emulation fools the computers into thinking that a monitor is attached.
I remember with my old KVM without it, the displays will scramble as I
switched among CPUs because to the PC, it would look like the display had
been disconnected each time I switched.
Now with my current KVM with EDID emulator the cards are fooled into
thinking that the display is attached even as a switch around and the
display always remain the same.

Just something more to think about....
