I have actually had that problem. Many years ago I was trying to buy laptops from an HP store and ended up going back in person to find out why they weren't answering my email. The guy said my email was empty. The email client (likely Outlook) only showed the top of the email which was the part I was replying to, so they thought I didn't send anything... There have been a few other cases since then.

It's also a problem with automated systems that only take the top part of a reply up to the start of the sent email.
