If it's a setting that you can change, setting it to "yes" is advisable.
However, I'm guessing that "no" refers to the state of the system (that it's
currently not in sync with whatever it considers the NTP servers).
Regardless, check and see how much the server's time differs from your phone's
time (estimate how much they're out of sync). This is what is most important.
Use the 'date' command from the server CLI to get it's current time, and
compare that with your phone's time (if you can't see how many seconds past the
minute your phone is, then check the server time when your phone rolls over to
a new minute).
Also, if you have the ntpdate command available to you, look for output similar
to the following (the IPs will differ, but the stratums should be 1, 2, or 3,
and the offsets should be well under 1/10th of a second).
$ ntpdate -q pool.ntp.org
server 217.180.209.214, stratum 1, offset -0.008099, delay 0.08443
server 129.250.35.251, stratum 2, offset -0.003139, delay 0.04790
server 73.239.145.47, stratum 2, offset -0.002151, delay 0.07530
server 108.61.73.244, stratum 2, offset 0.000496, delay 0.08293

Have you checked to confirm that your server and TOTP app times are within a
second of each other (e.g. synced to NTP)?
If they differ by by ~15 seconds, I would expect about half of the TOTP
verifications to fail.
