It happened again today with all third party extensions deleted. Next
I'll try additionally with all Ubuntu extensions disabled.

One thing I noticed is that it seems to happen if I boot without being
plugged into an external monitor, possibly if I had previously booted
while being connected to an external monitor.

I also noticed that restarting gdm3 would not work sometimes. Instead
I've been running this command, which not only seems to work, doesn't
require me to log in again:

killall -HUP gnome-shell

I'm attaching xrandr just for the laptop itself without being plugged
into any external monitors. I had the issue just this morning and ran
the xrandr command after killing gnome-shell.
