I was suffering from exactly the same problem. Identical output diagnostics. I have a different model of the hp slimline. s5360f. I had a fresh install of karmic koala. My workaround was to install the proprietary ATI/AMD FGLRX Video drive. My best guess why this worked is that the hdmi audio is integrated into this video card. Goto System -> Administration -> Hardware Drivers. It was the only driver on the list for me. This is only a workaround, I would prefer not to have to use proprietary drivers.
