I have a brand new machine, with Intrepid installed on it. My hardware combination triggers this bug.
After reading this thread, and many others similar to it, I have narrowed my problem down to the boot time interaction of ehci_hcd and my Alcor Micro Corp. Hi-Speed 21-in-1 Flash Card Reader/Writer. * Linux boxrd 2.6.27-9-generic #1 SMP Thu Nov 20 21:57:00 UTC 2008 i686 GNU/Linux * rmmod ehci_hcd -- works * kernel recompile with USB non-modularised -- doesn't work (was suggested elsewhere) * echo 64 > /sys/block/sdh/queue/max_sectors_kb -- doesn't work (and 120 also doesn't work) * rmmod uhci_hcd -- Doesn't work (besides disabling my USB keyboard - suggested elsewhere) * Unplug Bus 004 Device 002: ID 058f:6362 Alcor Micro Corp. Hi-Speed 21-in-1 Flash Card Reader/Writer -- works * Plug in Bus 004 Device 002: ID 058f:6362 Alcor Micro Corp.
