Why do you want this to be optional? The IMAP flags can only be manipualted by 
IMAP clients, so a decision based on flags is as safe as it goes in my book. 
Perhaps I'm missing something -- what is it?

And if you just block all images, you end up not protecting the users anyway as they will move to a mail client that will let them see the kitten pictures that people send to them without such horrid annoyances.

Leaving aside the question of what benefit such users would bring to us,  do 
you get a lot of HTML mail with images linked from remote sites? Most of the 
kitten mail I receive happens to come with embedded animals, not links. Perhaps 
my friend need further education here :).
