I see this somewhat frequently as well. It seems as if the order of strings are sometimes mixed up, and something is skipped. On some occasions, I can see an untranslated string in the context (above/below the current string) but it is never presented for translation until I restart from the overview page. I haven't been able to see a pattern, so I can't report anything reproducable.
