FWIW, I used to have directly connected speakers to the computer, and never 
had this type of problem.  When I bought new monitors, I changed to use the 
inbuilt USB speakers.  Now I have this same problem, especially for MS 
Teams.  I change the volume for the app, but stupid Windows doesn't 
remember them, and keeps setting it back to the device max.
