I can vouch for simscan.. :) I know both, but qfilter allows me to write my own filters, for example, block file types, filter Nigeria scam (wich gave me a big problem with my hosting company), etc. I have almost active 60000 users (I run a free mail service) and it's not so resource intensive as that.
