If you want to enable [SPAM] tag, use verbose_spamassasin as scanner.

I did but there is still no header added.
I have tested "spamc -f < sample-spam.txt >output", I got the header added in the output file.
If I switched back to use qmail 1.03, there is no problem. That is really strange.
