[toaster] File Error 6

I have installed the toaste, and everything works fine, but someone my staff
complians that they cannot access to QmailAdmin and it show out the error
is:
"File Error 6",  i don't understand what's problem, who else can help me to
slove it or give me a solution on it. As it's very annoying, sometime
happens sometime are ok. So strange.
