The short answer is that a fortress is the fortified city centre with its city walls and defensive towers, and a citadel is a stand-alone fortification inside or on the edge of it. A good example is Saint-Martin-de-RĂ© ( https://www.openstreetmap.org/#map=16/46.2026/-1.3657), where the https://www.openstreetmap.org/relation/308616 is the fortress and https://www.openstreetmap.org/way/35814333 is the citadel. https://wiki.openstreetmap.org/wiki/File:Vestingwerken_schema.svg shows essentially the same thing.
