Perhaps, but there are significant exceptions.

Sometimes people enter by driving into an underground carpark; or from
a metro station in the basement, or a walkway from another building.

Some buildings (or structures, more widely) have archways with no
door, or just a turnstyle, or are open to the outside along a whole
wall.

All of these, and more, need to be catered for.
