Ok, I may show my own stupidity here, but I'll give this a shot.

Sendmail is and MTA not a MDA. As far as I know, the MDA has to call
spamassassin (or spamd). This is where I may be wrong, or maybe this is just
the case with sendmail?

I use procmail as my MDA. I chose it because of added flexibility. IF you
implement procmail as your MDA for sendmail, then there is a script for it
in the spamassain docs. 

procmail delivers local email. So as a gateway this was a little tricky to
setup. each email is an alias to a procmail script on my system. It calls SA
then forwards to internal server. 

I believe I had read of another way of setting this up. I figured the
Sendmail Gateway -> exchange server would have been a popular choice. But
when I researched it, howto's and docs were in short supply.
