For me, a criteria in choosing a provider was the ability to provide a
subnet of static IP addresses *and* the provider had to support reverse DNS
for the block.  And while pricey, MegaPath does a very good job.  I was
trying to switch to Covad as they were cheaper and seemed to fit my
requirements.  But Covad could not handle the simple task of switching my
service.

One thing that I have not heard to help stem FPs is requiring the sending
host to helo/ehlo with a FQDN format.  I find I will kill a lot of broadband
direct connects as most directs seem to not want to helo in FQDN format.

I still don't understand why these "service" providers do not provide
service.  I hate telecom companies.
