Hi, Russell. If you check the rule itself in 20_dnsbl_tests.cf you'll notice the -notfirsthop part of the argument to check_rbl_txt(). This tells SpamAssassin to check all hops except the first one for this match. As a result it shouldn't match people sending email through their ISP's mail server, though I too am seeing a problem with some emails matching the DYNABLOCK rule despite being sent through their ISP's mail server. I had to go ahead and rescore the rule down to .1 because it was causing too much trouble (but I do want to see when it matches for diagnostic purposes).
