Running the milter as spamd user is nice. Create the user, make a homedir for it, edit the spamass-milter initscript so its started properly. Check sendmail settings. Last time I tried spamass-milter it wasn't really suitable for serious use because of crashing and processes blowing up to several hundred megabytes.
