FWIW, here is what I did on my Linux RedHat 7.2 system:

Download, configure and install MimeDefang 
(http://www.roaringpenguin.com/mimedefang) and SpamAssassin as well as all 
Perl modules required.

Upgrade Sendmail to version 8.12.5 or above to support milters. The 
MimeDefang program uses the plugin Mail::SpamAssassin instead of spamd. This 
method is very efficient and we are not experiencing any degradation of the 
system since starting MimeDefang over two weeks ago.

Used the "How-To" on the MimeDefang site to get it all up and running.

Add the "action_change_header" function call to my /etc/mail/mimedefang-
filter in the filter_end() section. This call changes the Subject header 
to "***SPAM***" along with the original Subject of the message.

I used the command "grep SPAM /var/log/<current name of maillog file> | wc -l"

Starting tonite at about 8:59 PM EDT I started the system with the default 
rules and the one change I made to rewrite subject headers. As of this 
writing I have caught over 1600 spam messages and flagged them for my 
subscribers to remove with a filter if they choose. This system works great 
and I am going to keep using it from now on. The impact to the system now 
that SpamAssassin has been added to the scanning process has been minimal so 
far.
