I had the same problem. I am using a qmail,vpopmail, qmail-scanner setup & SA setup, also with a 100% working local.cf file. I found the problem to be in qmail-scanner, if you are using it, edit your "qmail-scanner-queue.pl", look for the "$spamc_subject" line and insert your "****SPAM****" there, since I've done this,. it's been working 100%.
