First, check ALL the headers, including the Return-Path header. SA will whitelist on both and you've only included part of the headers in your report. Checking From: is not sufficient.

If you are SURE that none of the headers are a whitelisted address, can you replicate this on a non-development version, or at least a current devel version? You're running a daily snapshot from sometime prior to the official release of 2.43.

CVS snapshots generally work well, but sometimes introduce issues that hang around for a day and then get fixed. It would be impossible for me to know for sure that the version you checked out didn't have some temporary errors in it, since it could be from any day between the release of 2.42 and 2.43.. I don't *think* any such errors were introduced in that timeframe, but I cannot say for sure.
