I am having this same (I think) problem. 99% of emails get scanned, but a few don't each day. No explanation in the header of the email. I can see it passed through the machine running SendMail that SA runs on, but nothing in the headers about being 'touched' by SA.
 
While I don't have a solution for you, I though you might be interested to hear I had the problem too.

Just further to this, I disabled dcc checks by a) setting dcc_body to 0
then I disabled dcc completely in 20_body_tests.cf by commenting it
out. However I wass still noticing that some emails where not being
tested, so I switch SA to -L and now every email is tested,

On so possibly it is a system problem. SA is running under perl 5.05, using
the spamd/spamc via Exim on a P133 with 32mb of Ram. the PC is only used for
Email and nntp, no users actually log onto it.
