That's ok if you add a rule that guarantees that mail is not flagged as spam on the way out, since then the subject will not be changed. I don't have qmail or qmail-scanner, so I'm just guessing, but this sounds like you need to add the -F 0 option to your call to spamd. Without that option Spamassassin adds an mbox style From line to the beginning of each message.
