That's ok if you add a rule that guarantees that mail is not flagged as spam on the
way out, since then the subject will not be changed. You want to be sure to have such
a rule because otherwise there will be false positives on outgoing mail, at least
until the next version of qmail-scanner comes out with the option of not scanning
outgoing mail.

I don't have qmail or qmail-scanner, so I'm just guessing, but this sounds like you
need to add the -F 0 option to your call to spamd. Without that option Spamassassin
adds an mbox style From line to the beginning of each message. I can't imagine that
can be correct when you are filtering to something other than the final stages of an
mbox delivery agent.
