AH! Found it!

I had -S (stop when spam threshold reached) enabled on spamd. 

Problem is that -S pretty much makes whitelists useless. The whitelists
probably need to be checked early on in the process, cause with -S
enabled, once it hits the threshold, it will never bother to even check
the whitelist or blacklists.
