So I figured this out. It was a brower plugin that popped up a
notification every once in a while...problem is, I didn't see the
notification, but did see the after effect. Notifications...bah!

Is there a plan to show file and folder paths in the lenses? I am
sure I am not the only one to have multiple files and folders of the
same name in different paths. In the Dash lenses there seems to be no
way of identifying which one is which. Or perhaps it is so obvious
that I am missing it.

I have seen this white rectangular ghost image appear on my screen as well.
Next time I see it happening I will report a bug, but it is kind of hard to
describe since I don't know what it is caused by. You can remove the ghost
image by restarting unity via "unity --replace" for example.
