Since a moth (July 2015) I am having the same problem. (Up to now everything
worked pretty well,...4 years)
Randomly the Ubuntu freezes ( no ctr+atl+F1 or ssh) works). completely frozen.
Sometimes takes 1 hr sometimes 10 minutes.
I am not sure if it is because the graphics driver.

I am using a ThinkPad T420 with nVidia vidoe card ( NVIDIA Corporation
GF119 [Quadro NVS 4200M] (rev a1)) The drivers are the ones from the
package xserver-xorg-video-nouveau.
When I close the lid, sometimes I am logged out and all of my
applications are forcefully closed. A

experienced no crash after 6h.

outside helps:

For what it's worth, my problem was fixed when I bought a video card
(ATI Radeon 5570) to replace the onboard video. I never saw the problem

This happens to me as well. I use the dvorak and US layout. Also, when
doing '+ command', it only does it in the system default keyboard
layout. For example, in the US layout, the letter C is actually
