I would say that we should use it for all of the "major" applications.
I wouldn't use it for anything on the launcher, except maybe things
that are Canonical (or Ubuntu) related -- like Ubuntu Settings and
Ubuntu One, but not necessarily for Libre-*.
Then, I can work through adding \index tags.

IMO:
The use should be limited (so as to make the index useful). I would agree
with John that it should be limited to the section that describes the
application (if there is one), and then only in sections where the
application is integral to the topic at hand.

We shouldn't use the code on every app name we find for sure. I guess we
should use the code on apps that are relevant to the section they are in.
