This problem still exists for me (Mythbuntu Intrepid AMD64) in lirc
0.8.3-0ubuntu2.
I chose the Dvico USB IR as part of the install configuration.
Editing /etc/lirc/hardware.conf to set REMOTE_DEVICE to /dev/usb/hiddev0
(was /dev/lirc0) fixed the problem.

Whatever fix was implemented does not work for me: Maverick (rhythmbox
0.13.1).
Not only do additional formats appear in the "edit" list, but when a new
format is created - it is not available in the "Preferred format" list
either.

This bug also affected my system (exact same symptoms) but upgrading

I'm the original poster of this bug. In #6 I stated that the bug was
fixed in kernel 3.4.1. I was mistaken - it just became more
intermittent. I guess I was not following the situation closely, but
lately I seem to be getting 500+ a day. Note that the computer is not
always on.

Tested kernel Linux 3.4.1-030401 (sorry - I didn't see your link to
3.5-rc1 until after I downloaded 3.4.1). The issue is fixed with this
kernel. I rebooted with 3.2.0.24 and confirmed the issue is there with
that kernel.
