On a pretty fresh Ubuntu 16.04, I'm installing some Python packages with
pip (as a user). (pip version is 8.1.1). My example package is ipython.
The command:
$ pip install ipython
generates a bunch of downloads, including some packages that I
previously installed with aptitu
I can recreate the issue following the steps in the initial description.
Interestingly there are some (rare) cases where things works on 4.8 as
well. There is a difference in udev messages:

Note that this is not reporting these errors on all files accessed
during boot (by a long way) so we need to understand what is special
about those. I believe this is a MAAS boot and may be using bind mounts

Many GNOME apps have a new Keyboard Shortcuts popup window available in
their app menu (this features was introduced in GTK 3.20).
Ambiance and Radiance don't style this window correctly - there's no

I have just done some more test together with Alan Pope. Although I
first saw this happening on my M10FHD, Other devices also are affected:

FYI - I got to pass all other bits of my DPDK tests and got to the point where
this is recreated.

Official support for Ubuntu 10.04 "Lucid Lynx" and Ubuntu 12.10 "Quantal
Quetzal" has ended. If this issue is still occurring on Ubuntu 16.04
