I've got the same with 14.04 amd64.
I used hd-media boot.img with Easy2Boot USB hdd. There is a lot of images on
(more than 10) disk, but installer fails to scan them. If I open console with
Alt+F2, the partition with images already mounted in /hd-media like as AndreiF
wrote. Mounting image to /
Got the same problem with 11.10 and 12.10 (only tried i386).
Got around by replacing /var/lib/dpkg/info/iso-scan.* in 12.04's initrd.gz with
both files from 10.04's initrd.gz

I got around the problem by using initrd and vmlinuz extracted from the
iso, booting off it and then finally during the installation process
manually mounting the iso to /dev/cdrom. Finally I was able to installed
ubuntu 12.04 LTS!

images, after iso-scan 10 of them assigned to /dev/loop0-9.
