This turned out to be a bad /etc/default/grub config and has since been
resolved.

One other thing I might add,
When I find the system is not booting, the first thing I do is to re-
install grub with a trusty live usb. The first command I have was a
wrong one, which is `grub-install --force --boot-directory=/mnt/boot
/dev/cciss/c0d0p1` and that will install into first partition
We have spare identical hardware arriving shortly in Lexington. We plan
to try reproducing the problem on this system.

Colin, Ike was our remote hands. Unfortunately, it's a production system
so we're not able to try re-upgrading and rebooting. I'll see if we can
try replicate it elsewhere with the same hardware; I can't make any
promises though.

I am in front of that server when we try to find out the root cause.
With latest grub, there is nothing output on screen after "Attempting
boot from disk". Ctrl-alt-del is no function so I have to cold restart
the system.
After downgrade, system works normally and lots of kernel output after
bios
There's not much of a hint here as to how far GRUB might have got, which
makes it rather difficult to guess at the problem. Were there any
visual clues at all?
Can you try re-upgrading, working out what the usual grub-install
invocation is, adding --debug-image=all to it, then rebooting and seein
