One other thing I might add, When I find the system is not booting, the first thing I do is to re- install grub with a trusty live usb. The first command I have was a wrong one, which is `grub-install --force --boot-directory=/mnt/boot /dev/cciss/c0d0p1` and that will install into first partition not MBR. After reboot, system into grub rescue prompt and says file missing.
