I think I have found the solution:
Set org.gtk.Settings.FileChooser.LocationMode to "filename-entry"

Same problem here.
Solved by installing qml-module-qtquick-controls:
sudo apt -y install qml-module-qtquick-controls
Fix seems like adding a mandatory dependency.

totem. Never happenned before.

It seems to have disappeared for me too (firefox 3.0.3 Ubuntu).
I could reproduce it by killing my X server with an open firefox in it.
