I get this error on a new minimal VM install with all LVM disks except
'/boot':

A workaround for this that we're using now is to put the sysctl
processing into crontab after reboot.
e.g.
@reboot root sleep 5 && sysctl --system
Maybe one day someone will fix this bug :)

Forgive my ignorance, but what is a "d-i based install image"?
I'm also very troubled that an LTS release has these types of problems
with the installer. Is there a workaround to install directly to
existing LVM volumes? Specifically, I'm interested in using LVM volumes
not contained in a partitio
I can confirm that this is also happening in the latest version of
Ubuntu 16.04.

I had this issue also. In my case, there was a glitch at some point
during a fresh installation. Even though the root drive on my laptop
/dev/sdaX was where / and swap were set up, the fstab file listed the /
partition as mounted on /dev/sdb1. i have no idea how the system managed
to boot with this
I initially got this error on my laptop (DELL inspiron mini 1012) when
testing the Maverick RC, but I guessed that it was related to an Intel
video driver issue. Upon installing Maverick on my desktop, the audio is
