I've seen this happen on servers that run java webapps; it seems that
high java heap usage (especially when heap size is close to physical

- firmware is not loaded by the netbooted installer in Karmic and in
Lucid. I had to force it with "initrd=ubuntu-installer/amd64/initrd.gz
,ubuntu-installer/amd64/aic94xx-seq.gz" hack. I didn't know about scsi-
firmware udeb; looks like it's not workin
I'm not sure which package generates initrd.gz, but debian-installer is
my best guess.

This still won't give full serial console access (BIOS and GRUB won't be
configured).
In Karmic, replacing VGA BIOS with Google's SGABIOS works, and GRUB can
be configured in --execscript. In Lucid, these workarounds don't work
anymore (the latter is bug 703959).
