I discovered this when I organised my menus into logical categories
instead of the menus been monolithic.
To reproduce try the following;
Create a new menu called Multimedia in System > Preferences
Then copy over the following items from within Preferences into the new
Multimedia menu;

I too have encountered cursor issues within xinerama on a dual screen
setup (2x Compaq 1501 @ 1024x768 resolution).
