I am having the same problem on 16.04, non-virtualized.
For me the problem started when I switched from a motherboard with
Realtek RTL8111H to one with Intel I219-V. There were no other hardware
changes at the time and only regular software upgrades from Ubuntu.

I tried adding a debug log print to /lib/udev/hdparm to see which device
name is used. Looks like it is never called for ATA drives.
Using device names under /dev/[hs]d[a-z] is still quite bad, as the
device names may change.

There are two problems with the udev script (the one in 9.04):
1. It needs hdparm.conf to have the exact same device name as the one
seen by udev ([sh]d[a-z]). Using symlinks under /dev/disk is not
possible.
2. It seems to be never called for SATA drives.

As far as I can tell, there are at least three different problems mashed
together in this thread.
The first one is that the upstream M-audio firmware loader tries use the
old /proc/bus/usb interface. There is already a patch at

For me the problem is that the firmware loader can not open the device
without the strange wait. No path problems after the patch. I have
ACTION=="add", SUBSYSTEM=="usb", ENV{PRODUCT}=="763/2806/*",

When I look what happens when I plug in the sound card using udevmonitor

I have the same problem here with Radeon 9550. I fixed it by booting
with Live CD, chroot to my root partition and aticonfig -f --initial
--overlay-type=Xv.

Direct rendering does not work with ATI Radeon 9550 and proprietary
