In my case, I'm using Colemak layout for english and Persian layout for
typing in Farsi.
When in Colemak (en) I can use normal shortcuts (say I can use Ctrl+O to
open files and since I'm using colemak, I use the key with the ";" sign

menu pops up (such as using the menu bar or the overlay from Firefox's
address bar) the shell appears over the VLC fullscreen view and goes

On a fresh install of 13.10, the second display fails to detect /
initialise after X has started.
Installing a mainline kernel (It was an Intel drm-next nightly) resolved
the issue, and the second display detects and is useable as expected.

Just a follow up... My problem is fixed or at least figured out.
Although many with this bug have similar log output mine was definitely
hardware. SATA hard drives have extremely fragile connectors and the
connector to this particular drive with the read only problem was
slightly pushed down by an
Status changed to 'Confirmed' because the bug affects multiple users.
