It's just a guess, but I think most SIP providers aren't going to like
the all-zeroes Caller ID/ANI that you're sending here:

Mine is to use tcpdump:

Sounds like something is wrong with your invite request that these providers
aren't liking. You should ask them what it is. Or you can post the invite and
see if someone here can figure it out.
