This is not a GRASS problem since GRASS does not
enforce window placement. It's related to the
graphical user interface (KDE, gnome whatever).

This is not a bug.
Either you need to define a table for the new map within
v.digit (settings) or deactivate the attrib support
(lower left button).
One could consider to deactivate the button but then all
other users complain that GRASS does not ask for attributes...

AFAIK the "next not used" could also be an ID (cat)
with lower value than the current which would not
be "Auto increment" but "Auto decrement".

GRASS is not doing placement.
I assume that you mean size: in fact the window exceeds
800 pixels in widths which should be somehow fixed (smaller
icons?).
