Have you verified that /usr/local/apache/libexec/libphp4.so actually exists?
If it doesn't, PHP was not compiled and linked with apache correctly.
When you compiled PHP, did you use --with-apxs=/usr/local/apache/bin/apxs ,
so it would be compiled into Apache?
