Hi Bruce,
the error_reporting level in your php.ini is set to 81, that means only
a few sorts of errors will be shown on your site. Even parse-errors are
hidden.
But you can set it to a different level in your script.
Have a look at error_reporting in the manual.

There must be some headers somewhere because the mail has been sent to
you -> to header.
Have you clicked on the email in Outlook and looked at the headers / message

This is indeed not a php problem, but a browser
feature. The browser forces a reload whenever you have
a '?' in your code! (unless you use some proxy..).
I would also like to know how to overcome this - if
