For what it's worth, I also think this has been an issue with Visual Studio
in general and is not specificity a Mono issue.
Same problem I've faced many times, sure would be nice to have a 'make
clean' feature, which helps identify and stripped rogue assets! Idea for a
tool there...

Just managed to sort this out. Couple of rogue settings in project file
regarding build outputs.
Ended up removing the settings then creating a new solution and re-adding
existing projects.
