“in-reply-to”; it’s been a while since I’ve looked at the RFCs all but I
remember that this sort of explicit stuff can help with threading.

Another vote for fastmail. I won't argue whether you or not you need a
"dedicated server" for your mail - but I will say that fastmail does a lot of
mail for a lot of people.
I use Gandi.net for DNS domain registry and name service. They are definitely
not the cheapest but I'm a big fan of the
beta.

One thing of note. They are an Australian company, but from what I can tell
servers are in the US. Possibly automagically located close to you, but they
don't publish any way to pick an alternative AFAICT.

Oddly enough, I switched to Mailmate because of similar issues with Postbox.
Their support thought my mailboxes were too big. Even after I totally
recreated my profile, it got awfully slow.
Mailmate has been blazingly fast; I have experienced no slowdowns like you
