I had the same issues when I installed Mavericks,. What you have to do is turn
off VoiceOver right before you click on the create user button. You have to
make sure your pointer is on the button, turn off VO, press space bar or click
mouse, and turn on VO. That's how apple support walked me th

No, this is a known bug. I reported it to Apple hours after Mavericks was
released. You can use the terminal app as a work around or have sighted
assistance. This is only an issue when Voice Over is running. Really makes you
question apple's VO QA team.

Maybe, if you have sighted help at hand turn off Voiceover and have them click
on the appropriate buttons etc.
This is just a suggestion though to see if it crashes without Voiceover on. If
it doesn't then its obviously a bug with Voiceover or Maver
