Try adding
\paper {
\context {\RemoveEmptyStaffContext}
}
to the bottom of your \score block.
I couldn't test on 2.2.1, I am afraid, but see if it works.

Lily allows us to engrave music for two pianos, correct? I believe I did
everything correct to make lily print two piano staves, but instead of
showing 2 piano staves, it only shows piano staff 2. I must have made a
silly mistake, but I can't find it.

I want to use the "prallup" symbol ("scripts-prallup"), so that it is
_left_ to the note and "inside" the stave.

In normal music, you have
\context Staff{ ... } or \new Staff{...} and set the midi instrument
using \set Staff.midiInstrument="xxx" .
For chords, you have \context ChordNames{...} or \new ChordNames{...}
so why not try \set ChordNames.midiInstrument="xxx" ?

When I use lilypond on a .ly file, a nice postscript font is used.
When I use lilypond-book+latex+dvips on a .tex file however, a bitmap
font is used.
What should I do to have the nice font instead of the bitmap font ?
